When your patient leaves your facility, our work begins. We focus on Transitional Care, bridging the gap between acute settings and the home environment to prevent the "revolving door" of hospitalizations.

24/7 Support: A Haven caregiver can be available around the clock to address patient concerns, preventing unnecessary ER visits.
We Accept Referrals For Personal Care, which includes Aide services for hygiene, mobility, and ADL support.
Service Area: Accepting patients in Summit, Stark, Portage, Wayne, Trumball Mahoning, and Columbiana Counties counties.
We prioritize rapid response. For transitional care cases, we aim to admit patients within 24-48 hours of discharge to ensure safety.
A Haven clinician personalizes the plan based on your orders and the initial home assessment. We fax this plan to you for approval and provide regular progress summaries every 60 days (or as status changes).
Yes. Under CMS guidelines, patients must remain under the care of a physician. We act as your eyes and ears in the home, updating you on complications or goals achieved.
